Where can I find a sample of a copyright photo release form for my music artist?

Is this a simple form to fill out or would I need to consult a copyright attorney? Does the form need to be filed or notarized? Thanks!

Are you talking about a model release form, or a copyright assignment form?
Most forms will be simply retained by the two parties without having to be filed anywhere, as it is just an agreement between two parties. The signed forms can be produced in court, if this becomes necessary.
If the purpose of the form is to transfer a copyright from one person to another, while the Copyright Office does not provide such forms, they do record copyright transfers, if you send them a copy of your agreement.
